The first step is for the patient or their family to reach out to us set up a free consultation. Hospice benefits are fully covered under Medicare, Medicaid , Medi-Cal and most private insurance plans. We will work with you to understand your insurance benefits, as well as answer any other questions you have.
CONSULTATION
Your Free initial consultation can be done in person or over the phone. For in person consultations our care provider will come to you. We will assess your needs and answer any questions you might have. Knowing your wishes and preferences allows us to tailor our care program to your needs.
FIRST VISIT
During your first visit, our nurse will thoroughly analyze all your needs. We will create a personalized care plan tailored to your preferences, and provide education and support to you and your family. We want to ensure that your first visit is a welcoming and supportive experience.
ONGOING VISITS
The cadence of visits depends on the specific needs of the patient. We will make visits as frequently as needed to provide the best care, considering doctor’s and interdisciplinary team’s approval, as well as patient’s wishes.
