Our hospice Medical Doctors provide gentle, attentive medical care focused on comfort and peace at the end of life. With experience in hospice and palliative medicine, they guide care with compassion, ensuring pain and symptoms are carefully managed so patients can rest as comfortably as possible.
Working closely with nurses, the hospice team, and the patient’s attending physician, our doctors help create a plan of care that reflects each patient’s wishes and goals. They are also a source of support and reassurance for families, offering guidance, clarity, and understanding during this deeply personal time.
Administrators
Our administrators work behind the scenes to support every aspect of hospice care at Life Source Hospice. They ensure that services are delivered smoothly, ethically, and in accordance with Medicare hospice regulations, allowing our clinical team to focus fully on caring for patients and families.
From coordinating services to maintaining compliance and quality standards, our administrative team helps create a caring and well-organized environment. Their dedication ensures that patients and families experience hospice care that is respectful, timely, and centered on comfort and dignity during the end of life.
Director of Nursing
The Director of Nursing provides leadership and clinical oversight for our hospice nursing team. With a strong commitment to quality and comfort at the end of life, the Director of Nursing ensures that all nursing care meets hospice standards and Medicare requirements. They support and mentor nurses, participate in care planning, and work closely with the interdisciplinary team to ensure care is delivered with consistency, skill, and heart.
Clinical Case Managers
Clinical Case Managers play a central role in guiding hospice care for patients and families. They conduct ongoing assessments, help develop individualized plans of care, and coordinate services across the hospice team. Serving as both advocates and educators, Clinical Case Managers help ensure that care aligns with the patient’s goals, comfort needs, and wishes, while also providing support and clear communication for loved ones.
Visiting Registered Nurses (RNs)
Visiting Registered Nurses provide skilled, compassionate nursing care in the patient’s home. Their focus is on comfort, pain and symptom management, and ongoing assessment during the end of life. RNs administer medications, monitor changes in condition, and provide education and emotional support to patients and families. They work closely with the hospice physician and care team to ensure needs are met promptly and with sensitivity.
Visiting Licensed Vocational Nurses (LVNs)
Visiting LVN Nurses support patients by providing skilled nursing care under the direction of the hospice physician and Registered Nurse. They assist with symptom management, medication administration, and monitoring patient comfort. LVNs are an important part of the hospice team, helping ensure continuity of care and offering compassionate presence to patients and families during this time.
Visiting Caregivers and Certified Home Health Aides (CHHAs)
Certified Home Health Aides and visiting caregivers provide hands-on assistance with personal care and daily activities, helping patients remain comfortable and dignified at home. They offer bathing, grooming, light assistance, and meaningful companionship. Working under the supervision of the nursing team, CHHAs and caregivers provide respectful support that helps ease both physical and emotional burdens at the end of life.
Additional Teams Supporting Hospice Care
Chaplains
Chaplains provide spiritual and emotional support to patients and families during the end of life. They offer a compassionate presence, listen without judgment, and support individuals as they reflect on meaning, faith, hope, or personal beliefs. Chaplain services are available to people of all faiths and spiritual backgrounds, honoring each person’s values and wishes during this sacred time.
Wound Care Team
Our Wound Care Team provides specialized attention to skin integrity and wound management when needed. Their focus is on comfort, prevention of complications, and gentle care. Working closely with the hospice nurse and physician, the team helps ensure wounds are managed in a way that supports dignity and overall comfort at the end of life.
Social Workers
Hospice Social Workers support patients and families with the emotional, social, and practical challenges that often arise at the end of life. They provide counseling, assist with advance care planning, and help families navigate difficult decisions. Social Workers also connect families with community resources and provide ongoing emotional support throughout the hospice journey.
Pharmacists
Our hospice pharmacists work closely with the medical team to ensure medications are used safely and effectively to manage pain and symptoms. They review medications, help prevent side effects or interactions, and support comfort-focused care. Pharmacists also provide education and reassurance to patients and families regarding medication use during hospice care.
Bereavement Counselors
Bereavement Counselors offer ongoing support to families after the loss of a loved one. Hospice bereavement services are provided for up to 13 months following the patient’s passing and include counseling, grief education, and supportive resources. These services help families navigate grief with compassion, understanding, and care.
Volunteer Coordinators and Hospice Volunteers
Hospice volunteers offer companionship, emotional support, and respite for caregivers. Volunteer Coordinators oversee training and ensure volunteers are matched thoughtfully to patient and family needs. Volunteers bring comfort through simple acts of presence, helping create moments of connection, kindness, and support during the end of life.
